All your responses will be kept confidential and you will not be identified.  This survey should take about 15-20 minutes.

Study Detail

The purpose of this research study is to learn about the self-reported health and well-being of COVID-19 healthcare workers and others who work in healthcare settings or in communities to support the care of COVID-19 patients across the United States.

If you agree to participate you will complete a survey that will ask about you, your job role(s), your workplace environment, your health, and your well-being, and share your experiences during the COVID-19 pandemic. You will also have an opportunity to participate in a follow-up interview and future studies related to the CHAMPS study.  The alternative to participating in the CHAMPS study is not to participate. You will not be paid to take part in this study, and you will not have to pay to participate either.

All of your responses will be kept confidential including your participation in the study and your work site. Only members of the research team analyzing the data will be able to view individual responses. Individual responses will be combined with those from other participants to describe the health and well-being of all who participated in the study.  The information you provide will be used only for research purposes. This research is covered by a Certificate of Confidentiality from the National Institutes of Health to protect your answers from disclosure.
